Rates
$170 for the initial intake session
$135 for subsequent standard 45 minute therapy sessions
$160 for subsequent longer 60 minute therapy sessions
Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. I am a preferred provider for several different insurance companies and EAPs (Employee Assistance Programs). If I’m not a provider for your particular company, most PPO plans provide some reimbursement for out-of-network providers. I can provide you with a receipt that has the information needed to apply for reimbursement.
Please check your coverage carefully by contacting your insurance company and asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- Do I have a co-pay?
- Is this therapist considered an in-network provider? If not, is there coverage for out-of-network providers?
- Is prior authorization required?
Payment
Payment is due at the time of service. Payment can be made by cash or check.
Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay a fee of $50.00.
